Food Express, a takeaway at 315 Higham Hill Road, Walthamstow was given the score after assessment on July 11, the Food Standards Agency's website shows.
It means that of Waltham Forest's 240 takeaways with ratings, 83 (35%) have ratings of five and 11 have zero ratings.
