Carolines Pizza, Bravo Pizza, a takeaway at 31 Burdett Road, London was given the score after assessment on September 22, the Food Standards Agency's website shows.
It means that of Tower Hamlets's 382 takeaways with ratings, 242 (63%) have ratings of five and just one has a zero rating.
