Spices republic ltd, a takeaway at 499, Lordship Lane, London was given the score after assessment on October 6, the Food Standards Agency's website shows.
It means that of Southwark's 463 takeaways with ratings, 242 (52%) have ratings of five and just one has a zero rating.
