Japanese yama sushi, a restaurant, cafe or canteen at 6, Trinity Street, London was given the score after assessment on March 15, the Food Standards Agency's website shows.
It means that of Southwark's 972 restaurants, cafes and canteens with ratings, 664 (68%) have ratings of five and just one has a zero rating.
