Piazza Firenze, a restaurant, cafe or canteen at 133 High Street, Hampton Hill was given the score after assessment on April 5, the Food Standards Agency's website shows.
It means that of Richmond's 486 restaurants, cafes and canteens with ratings, 339 (70%) have ratings of five and just one has a zero rating.
