Chicago 30, a takeaway at 369 Katherine Road, Forest Gate, London was given the score after assessment on October 5, the Food Standards Agency's website shows.
It means that of Newham's 313 takeaways with ratings, 123 (39%) have ratings of five and just two have zero ratings.
