Waffle Jack's, a takeaway at 8 Merton Park Parade, Kingston Road, Wimbledon, London was given the score after assessment on September 1, the Food Standards Agency's website shows.
It means that of Merton's 123 takeaways with ratings, 72 (59%) have ratings of five and none have zero ratings.
