Beef Belly, a takeaway at 14 New Broadway, Uxbridge Road, Uxbridge was given the score after assessment on July 5, the Food Standards Agency's website shows.
It means that of Hillingdon's 241 takeaways with ratings, 128 (53%) have ratings of five and just one has a zero rating.
