Seven Seas, a takeaway at 312 Munster Road, London was given the score after assessment on June 29, the Food Standards Agency's website shows.
It means that of Hammersmith and Fulham's 128 takeaways with ratings, 83 (65%) have ratings of five and just one has a zero rating.
