The Cuckoo, a restaurant, cafe or canteen at 85 Greenford Avenue, Hanwell, London was given the minimum score after assessment on July 14, the Food Standards Agency's website shows.
It means that of Ealing's 718 restaurants, cafes and canteens with ratings, 290 (40%) have ratings of five and just 13 have zero ratings.
