Dartbrooke Coffee, a restaurant, cafe or canteen at 77 Coleman Street, London was given the score after assessment on May 18, the Food Standards Agency's website shows.
It means that of City of London's 760 restaurants, cafes and canteens with ratings, 652 (86%) have ratings of five and just one has a zero rating.
