Musthafa Kitchen, a takeaway at 148 Willesden Lane, Kilburn, London was given the score after assessment on September 12, the Food Standards Agency's website shows.
It means that of Brent's 396 takeaways with ratings, 252 (64%) have ratings of five and just two have zero ratings.
